GP Practice Profiles

In 2017 there were 81 GP practices in Gloucestershire. The GP practices are overseen by the Gloucestershire Clinical Commissioning group (CCG). The geographical area covered by the CCG is coterminous with the county boundary. The CCG is split into seven GP Localities; Cheltenham, Forest, Gloucester, North Cotswold, South Cotswold, Stroud & Berkeley Vale and Tewkesbury. These differ from the district boundaries. Data in these profiles are presented by GP Locality in order to inform service planning and in recognition that individuals may not necessarily be resident in the same locality in which their GP Practice is located. Each locality has a GP liaison lead sitting on the CCG Governing Body and a Locality Executive Group where ‘local’ decisions can be made.

All 81 practices have been aggregated into 9 taxonomy groups using the most common age group, deprivation level and prevalence of disease of their registered patients.

Data presented in these profiles are based on GP Practice disease registers (maintained under the Quality and Outcomes Framework).

Prevalence is a measure of the recorded frequency of a disease or health condition in a population at a particular point in time. Recorded prevalence depends on a number of factors:

  • Individuals need to be registered
  • Present with symptoms
  • Receive diagnosis
  • The diagnosis has to be recorded and added to the register

The profiles are presented for the following topics: cancer, circulatory diseases, diabetes and chronic kidney disease, mental health conditions and dementia, musculoskeletal conditions, respiratory conditions, and wider determinants - lifestyle and deprivation.

 

Cancer GP Practice Profile

This profile covers:

  • Percentage of patients with cancer
  • Percentage of eligible patients attending cervical cancer screening
  • Percentage of eligible patients attending breast cancer screening
  • Percentage of eligible patients attending bowel cancer screening

 

  • Percentage of patients with coronary heart disease
  • Percentage of patients suffering from stroke
  • Percentage of patients with heart failure
  • Percentage of patients with atrial fibrillation
  • Percentage of patients with hypertension
  • Percentage of newly diagnosed hypertension patients being treated with statins

 

  • Percentage of patients with diabetes
  • Percentage of patients with chronic kidney disease
  • Last BP reading measures in last 12 months is <+140/85 (2014/15)

 

  • Percentage of patients self reporting poor mental health
  • Percentage of patients diagnosed with severe mental health conditions and dementia
  • Percentage of patients with depression
  • Percentage of depression patients who were diagnosed in the previous financial year
  • Percentage of patients with dementia

 

  • Percentage of patients with osteoporosis
  • Percentage of patients with rheumatoid arthritis

 

  • Percentage of patients with chronic obstructive pulmonary disease (COPD)
  • Percentage of patients with asthma

 

  • Percentage of patients aged 0-4 years (2016 mid year estimates)
  • Percentage of patients aged 65+ years (2016 mid year estimates)
  • Deprivation score (IMD 2015)
  • Percentage of patients who smoke
  • Percentage of patients aged 18+ years who are obese
